Inmediatamente surge la preocupaci\u00f3n: \u00bfes c\u00e1ncer? \u00bfnecesito cirug\u00eda? \u00bfvoy a perder mi tiroides?<\/p>\n

Tener m\u00faltiples n\u00f3dulos es mucho m\u00e1s com\u00fan de lo que piensas y en la mayor\u00eda de los casos no requiere cirug\u00eda. Pero s\u00ed requiere una evaluaci\u00f3n cuidadosa. Aqu\u00ed te explico todo lo que necesitas saber.<\/p>\n

\u00bfQu\u00e9 es un bocio multinodular?<\/h2>\n

Un bocio multinodular es simplemente una tiroides que contiene dos o m\u00e1s n\u00f3dulos. La tiroides es una gl\u00e1ndula en forma de mariposa ubicada en la parte anterior del cuello que produce hormonas esenciales para el metabolismo. Con el tiempo, especialmente en mujeres y en personas mayores de 50 a\u00f1os, es muy com\u00fan que se desarrollen n\u00f3dulos en la tiroides.<\/p>\n

Para ponerlo en perspectiva, si hici\u00e9ramos un ultrasonido de tiroides a todas las personas mayores de 60 a\u00f1os, encontrar\u00edamos n\u00f3dulos en m\u00e1s del 50% de ellas. La gran mayor\u00eda de estos n\u00f3dulos son completamente benignos. Tener muchos n\u00f3dulos no significa que tengas m\u00e1s probabilidad de c\u00e1ncer,\u00a0 de hecho, el riesgo de c\u00e1ncer por n\u00f3dulo individual es similar o incluso menor en un bocio multinodular que en un n\u00f3dulo solitario.<\/p>\n

\u00bfPor qu\u00e9 se forman los n\u00f3dulos?<\/h2>\n

Los n\u00f3dulos tiroideos pueden formarse por varias razones. La deficiencia de yodo es una causa hist\u00f3rica importante, aunque menos relevante hoy en d\u00eda gracias a la yodaci\u00f3n de la sal. Los factores gen\u00e9ticos juegan un papel significativo: si tus padres o hermanos tienen n\u00f3dulos tiroideos, t\u00fa tienes m\u00e1s probabilidad de desarrollarlos. Las fluctuaciones hormonales, especialmente durante el embarazo, pueden estimular el crecimiento de n\u00f3dulos. Y simplemente el envejecimiento normal contribuye a su formaci\u00f3n.<\/p>\n

La evaluaci\u00f3n: \u00bfqu\u00e9 n\u00f3dulos merecen atenci\u00f3n?<\/h2>\n

Cuando tienes m\u00faltiples n\u00f3dulos, no es necesario ni pr\u00e1ctico biopsiar todos. La clave est\u00e1 en identificar cu\u00e1les tienen caracter\u00edsticas sospechosas. Esto se hace mediante un ultrasonido de alta resoluci\u00f3n, donde evaluamos cada n\u00f3dulo seg\u00fan criterios estandarizados como el sistema TI-RADS del Colegio Americano de Radiolog\u00eda.<\/p>\n

Los criterios que nos preocupan incluyen n\u00f3dulos s\u00f3lidos con bordes irregulares, la presencia de microcalcificaciones, n\u00f3dulos m\u00e1s altos que anchos, y n\u00f3dulos con ecogenicidad marcadamente reducida. Tambi\u00e9n prestamos especial atenci\u00f3n al n\u00f3dulo dominante, el m\u00e1s grande del grupo, y a cualquier n\u00f3dulo que haya crecido significativamente en evaluaciones previas.<\/p>\n

Si un n\u00f3dulo cumple criterios de sospecha y tiene al menos un cent\u00edmetro de di\u00e1metro, recomendamos una biopsia por aspiraci\u00f3n con aguja fina. Este procedimiento se realiza en el consultorio, guiado por ultrasonido, y es generalmente bien tolerado.<\/p>\n

S\u00edntomas compresivos: cuando el tama\u00f1o importa<\/h2>\n

Aunque la mayor\u00eda de los bocios multinodulares son asintom\u00e1ticos, cuando la tiroides crece lo suficiente puede comprimir las estructuras vecinas. Los s\u00edntomas compresivos incluyen dificultad para tragar, especialmente con alimentos s\u00f3lidos, sensaci\u00f3n de presi\u00f3n o masa en el cuello, dificultad para respirar especialmente al acostarse, y cambios en la voz si el bocio afecta el nervio lar\u00edngeo recurrente.<\/p>\n

Un tipo particular de bocio que requiere atenci\u00f3n especial es el bocio subesternal o retroesternal, donde parte de la tiroides se extiende hacia el t\u00f3rax detr\u00e1s del estern\u00f3n. Estos bocios pueden comprimir la tr\u00e1quea y los grandes vasos, y generalmente requieren tratamiento quir\u00fargico.<\/p>\n

\u00bfCu\u00e1ndo se recomienda cirug\u00eda?<\/h2>\n

Las indicaciones para cirug\u00eda en un bocio multinodular incluyen sospecha o confirmaci\u00f3n de c\u00e1ncer en alg\u00fan n\u00f3dulo, s\u00edntomas compresivos significativos que afectan la degluci\u00f3n o la respiraci\u00f3n, crecimiento progresivo a pesar de seguimiento, bocio subesternal e hipertiroidismo causado por n\u00f3dulos aut\u00f3nomos que no responde al tratamiento m\u00e9dico.<\/p>\n

La cirug\u00eda puede ser una tiroidectom\u00eda total, donde se remueve toda la gl\u00e1ndula, o una lobectom\u00eda, donde se remueve solo un lado. La decisi\u00f3n depende de la distribuci\u00f3n de los n\u00f3dulos, los resultados de las biopsias y otros factores individuales.<\/p>\n

Mi enfoque: vigilancia activa inteligente<\/h2>\n

En mi pr\u00e1ctica, creo firmemente en no operar innecesariamente. Si tus n\u00f3dulos son benignos en la biopsia, no causan s\u00edntomas y la funci\u00f3n tiroidea es normal, la vigilancia activa con ultrasonidos peri\u00f3dicos es el mejor abordaje. Esto significa evaluaciones cada 12 a 24 meses para monitorear el tama\u00f1o y las caracter\u00edsticas de los n\u00f3dulos. Si algo cambia, actuamos. Pero no hay raz\u00f3n para someter a un paciente a una cirug\u00eda que no necesita.[\/vc_column_text][vc_column_text css=\u00bb\u00bb el_class=\u00bbbutton-01″]<\/p>\n
