[vc_row][vc_column][vc_column_text css=\u00bb\u00bb]Por Dr. Laureano Giraldez-Rodriguez, MD, FACS <\/em>[\/vc_column_text][vc_column_text css=\u00bb\u00bb]Una de las preguntas que m\u00e1s me hacen los pacientes con n\u00f3dulos tiroideos benignos es: \u00bfme opero o existe otra alternativa? Y la respuesta, cada vez m\u00e1s, incluye la ablaci\u00f3n por radiofrecuencia, o ARF. Hoy quiero darte una comparaci\u00f3n honesta y detallada entre ambas opciones para que puedas tomar la decisi\u00f3n m\u00e1s informada posible.<\/p>\n

\u00bfQu\u00e9 es exactamente la ablaci\u00f3n por radiofrecuencia tiroidea?<\/h2>\n

La ARF es un procedimiento m\u00ednimamente invasivo que utiliza energ\u00eda de radiofrecuencia, b\u00e1sicamente calor controlado, para reducir el tama\u00f1o de n\u00f3dulos tiroideos benignos. Se realiza con una aguja especial que se inserta directamente en el n\u00f3dulo bajo gu\u00eda de ultrasonido, y la energ\u00eda t\u00e9rmica destruye gradualmente el tejido del n\u00f3dulo, que luego es reabsorbido por el cuerpo durante los meses siguientes.<\/p>\n

El procedimiento se hace con anestesia local, en la oficina o en un centro ambulatorio, y t\u00edpicamente toma entre 20 y 40 minutos dependiendo del tama\u00f1o del n\u00f3dulo. El paciente est\u00e1 despierto y puede comunicarse con el equipo durante todo el procedimiento.<\/p>\n

La cirug\u00eda tradicional: lobectom\u00eda o tiroidectom\u00eda<\/h2>\n

La cirug\u00eda de tiroides, ya sea remover un l\u00f3bulo (lobectom\u00eda) o toda la gl\u00e1ndula (tiroidectom\u00eda total), ha sido el est\u00e1ndar de tratamiento por d\u00e9cadas. Es un procedimiento que se realiza bajo anestesia general en el quir\u00f3fano, requiere una incisi\u00f3n en el cuello y t\u00edpicamente una estad\u00eda hospitalaria de uno a dos d\u00edas.<\/p>\n

La cirug\u00eda remueve completamente el n\u00f3dulo o la gl\u00e1ndula afectada, proporcionando tambi\u00e9n tejido para an\u00e1lisis patol\u00f3gico definitivo. Es el tratamiento indicado para n\u00f3dulos malignos o sospechosos de malignidad.<\/p>\n

Comparaci\u00f3n directa: punto por punto<\/h2>\n

En cuanto a candidatos apropiados, la ARF est\u00e1 indicada para n\u00f3dulos tiroideos benignos confirmados por biopsia que causan s\u00edntomas como presi\u00f3n, dificultad para tragar o preocupaci\u00f3n cosm\u00e9tica. La cirug\u00eda es apropiada para n\u00f3dulos benignos sintom\u00e1ticos, n\u00f3dulos malignos, n\u00f3dulos con biopsia indeterminada, y cuando se necesita un diagn\u00f3stico histol\u00f3gico definitivo.<\/p>\n

Respecto al procedimiento en s\u00ed, la ARF utiliza anestesia local, dura entre 20 y 40 minutos, no requiere hospitalizaci\u00f3n y deja una marca m\u00ednima de punci\u00f3n que desaparece. La cirug\u00eda requiere anestesia general, dura de una a tres horas, necesita hospitalizaci\u00f3n y deja una cicatriz en el cuello de tres a seis cent\u00edmetros que, aunque generalmente se disimula bien, es permanente.<\/p>\n

En resultados de reducci\u00f3n, la ARF logra t\u00edpicamente una reducci\u00f3n del volumen del n\u00f3dulo de 50 a 80% durante los seis a doce meses posteriores al procedimiento. La cirug\u00eda remueve el n\u00f3dulo o la gl\u00e1ndula por completo, con un resultado inmediato y definitivo.<\/p>\n

La recuperaci\u00f3n con ARF es r\u00e1pida: la mayor\u00eda de pacientes regresan a actividades normales en uno a dos d\u00edas. La recuperaci\u00f3n quir\u00fargica toma de dos a cuatro semanas para actividades completas.<\/p>\n

En cuanto a la funci\u00f3n tiroidea, la ARF preserva la gl\u00e1ndula tiroides y su funci\u00f3n, por lo que la mayor\u00eda de pacientes no necesitan medicaci\u00f3n de reemplazo hormonal. Despu\u00e9s de una lobectom\u00eda, aproximadamente el 20 al\u00a030 por ciento de pacientes necesitan reemplazo hormonal. Despu\u00e9s de una tiroidectom\u00eda total, el cien por ciento necesita levotiroxina de por vida.<\/p>\n

Riesgos y complicaciones<\/h2>\n

La ARF tiene un perfil de riesgo generalmente favorable. Las complicaciones m\u00e1s comunes son dolor leve durante o despu\u00e9s del procedimiento, hematoma en el sitio de punci\u00f3n y, raramente, cambio temporal en la voz si el n\u00f3dulo est\u00e1 muy cerca del nervio lar\u00edngeo recurrente. Las complicaciones serias son poco frecuentes.<\/p>\n

La cirug\u00eda tiene riesgos que incluyen lesi\u00f3n del nervio lar\u00edngeo recurrente que puede afectar la voz de forma temporal o permanente, hipoparatiroidismo temporal o permanente que afecta los niveles de calcio, hematoma posquir\u00fargico, infecci\u00f3n de la herida quir\u00fargica y riesgos asociados a la anestesia general.<\/p>\n

\u00bfCu\u00e1ndo recomiendo ARF y cu\u00e1ndo cirug\u00eda?<\/h2>\n

En mi pr\u00e1ctica, recomiendo ARF cuando el n\u00f3dulo es benigno y confirmado por al menos dos biopsias, cuando causa s\u00edntomas compresivos o preocupaci\u00f3n est\u00e9tica, cuando el paciente prefiere evitar cirug\u00eda y anestesia general, y cuando la anatom\u00eda del n\u00f3dulo es favorable para el procedimiento.<\/p>\n

Recomiendo cirug\u00eda cuando hay sospecha o confirmaci\u00f3n de malignidad, cuando la biopsia es indeterminada y se necesita diagn\u00f3stico histol\u00f3gico, cuando hay bocios multinodulares extensos, cuando hay hipertiroidismo que no responde a otros tratamientos, y cuando el n\u00f3dulo tiene caracter\u00edsticas que hacen la ARF t\u00e9cnicamente dif\u00edcil.<\/p>\n

Al final del d\u00eda, mi recomendaci\u00f3n siempre se basa en qu\u00e9 es mejor para cada paciente individual, considerando su anatom\u00eda, sus diagn\u00f3sticos, sus preferencias y su estilo de vida.<\/p>\n

Lo m\u00e1s importante es tener una conversaci\u00f3n abierta y honesta. No existe la opci\u00f3n perfecta universal. Existe la opci\u00f3n correcta para ti, y encontrarla requiere un di\u00e1logo informado entre m\u00e9dico y paciente.[\/vc_column_text][vc_column_text css=\u00bb\u00bb el_class=\u00bbbutton-01″]<\/p>\n
